**Physical forms are available at both the Grade School and the High School office. The parent/student history has to be filled out and signed by both the athlete and the parent. Incoming Freshmen do not need a sports physical BUT will need to have a full medical physical to enter grade 9, in accordance with the State of Illinois regulations.
